Youth Programs
Services

Special youth programs (ages 5 to 12 years) are offered at Christ Child Center.

 

Forms should be submitted either by mail or in person to 1248 South 10th Street, Omaha, NE 68108.

We require a current copy of your child’s immunization records, which may accompany your enrollment form or may be faxed to 402.342.4541 Attn: Cristina.

Other required forms will be distributed to parents after a completed enrollment form has been submitted.
